Read the Summer School Catalog CAREFULLY

**If you are unable to download any of the forms, please contact the summer school office to request a catalog and application materials.

*APPLICATION*

Fill out the 2006 Kapälama Campus Summer School Application Form completely. Incomplete forms will not be processed.
A few hints:

a) Be sure to list the child's social security number (SSN).
b) List alternate choices since your first choice may not be available.
c) Mark the appropriate boxes to request country bus service ($25 one way, $50 roundtrip) if desired. (Bus routes and information available in online catalog)
d) Make sure there are two signatures on the back of the form. If you are unable to get a second signature, circle the reason. If no reason is listed the form will be considered incomplete.

*MEDICAL FORM*

Please fill out the Kapälama Campus Summer School 2006 Medical Form. Two signatures are required for a complete application. If parent is single, divorced or a parent is deceased, please note. If Legal Guardian signs, please send copy of guardianship certificate or other legal documentation.

*FINANCIAL AID*

Financial aid covers the tuition for morning classes and bus only, not afternoon classes.

KS students currently receiving financial aid for 2005-06 school year
These students are already eligible for financial aid, and DO NOT HAVE TO complete a Financial Aid Application for Summer School.
ALL OTHER STUDENTS applying for financial aid (includes KS students NOT currently receiving financial aid and siblings of KS students)
MUST complete a Summer School Financial Aid Application.

**Note: Students who are awarded financial aid and later decide not to attend summer school must notify the Summer School Office immediately to allow other students on the waiting list for classes the opportunity to attend. Parents failing to inform the Summer School Office in a timely fashion, prior to or by the start of summer school, will be given lower priority in registering for future summer programs.

*PREFERENCE POLICY*

Kamehameha Schools’ policy is to give preference to applicants of Hawaiian ancestry to the extent permitted by law.

*HAWAIIAN ANCESTRY VERIFICATION*

KS has established the Ho'oulu Hawaiian Data Center (Data Center) to build a comprehensive database of the Hawaiian population to support planning for lifelong learning in the Hawaiian community. One of the Data Center’s roles is to verify the Hawaiian ancestry of applicants who wish to be considered under KS’ preference policy.

Students currently attending KS who have previously documented their Hawaiian ancestry do not need to resubmit verification.

All questions and documents regarding verification should be submitted directly to the Data Center. The Data Center requires original birth certificates and documents. Please do not submit any original birth certificates with applications to KCSS.

To request a HAR packet immediately, please call the Data Center at 808–523–6228, or toll–free at 1–800–842–4682, then press 9, then 36228. You may also visit the Data
Center at 567 S. King Street, Suite 130 in Honolulu. HAR packets are also available at the Community Learning Center in Nänäkuli and the KS Neighbor Island Regional

*Note: Parking is limited. Do not park in No Parking/Reserved spaces. There is no in-person registration. Registration materials will not be processed while you wait.

*PAYMENT*

During the first week of May all registered students will receive a bill. Payment must be submitted to the Cashier's Office. Payment may be made by check, money order, VISA or MasterCard. A $20 processing fee will be charged for any check returned by the bank. No payments will be accepted at the Summer School Office(s).

8)

*CHANGING COURSES*

Requests for course changes, refunds or withdrawals must be made in writing. Please fill out a Withdrawal/Course Change Form for your convenience and faster processing of your refund. You may fax your request to 842-0850. Should you withdraw from Summer School, the following feels will be charged:

  • Prior to May 12: $25.00
  • From May 13- May 29: $100.00
  • From May 30 - June 9: $200.00
  • From June 10: no refund

*CONFIRMATION*

After your child is registered and payment is received at the cashier's, a complete schedule, bus ticket and lunch ticket (if appropriate), will be sent to you the last week in May. We ask for your patience, please do not call to check on your child's registration. We will be able to send you confirmation earlier if we can focus on the registration process for all students.
