Health Requirements
Kamehameha Schools Kapalama Campus health requirements
are in compliance with the State of Hawaii’s Department
of Health standards.
Physical Examination:
All new incoming students and returning students entering Grades
4, 7, 9, and 11 are required to have a physical examination done
within one year prior to start of the incoming/returning school
year. The physical examination must be documented on the Kamehameha
Schools Student Health Record. Returning students entering Grades
1, 2, 3, 5, 6, 8, 10, and 12 are not required to have a physical
examination.
The Student Health Record and other medical forms are updated
yearly and are available at the beginning of April for the next
school year. These forms are due July 1 for the incoming/returning
school year. Failure to complete the required physical examination
and/or other required forms will prevent the student from entering
school and boarding.
TB Skin Test:
A
TB skin test is required for all new incoming students and returning
students entering Grades 4, 7, 9, and 11. If a student has a positive
TB skin test on the physical examination, a follow-up chest x-ray
is required. The results of the chest x-ray need to be documented
on the health form.
If a student has a known history of a positive TB skin test,
a follow-up chest x-ray result is required to be submitted with
the required physical examination. The chest x-ray needs to have
been done within two years of the start of the incoming/returning
school year. Results should be documented on the health form.
Athletics:
For
students who are planning to participate in the school athletics
program, a separate Athletic Participation Form must be completed
and submitted to the Athletic Department. Please note that a yearly
physical examination is required for participation in the athletic
program. This form and any questions regarding the school athletic
program can be obtained from the Athletic Department.
