Medical Services Department Kapalama Campus Health Records A health record is maintained on each student. The health record contains a chronological record of each visit to Hale Ola or Kalanimoku Dispensary, the Medical Care Permit (MCP), required physical examinations, and other health related documents. The information contained on these documents is very important as it contains emergency and O`ahu adult contacts, health insurance information and immunization records. This information must be updated annually and any changes must be reported to Hale Ola promptly at 842-8075. Due to the many restrictive health insurance plans available today, the MCP includes pertinent information necessary for the Medical Service Department to ensure the student obtains care from providers who participate with your health plan. KS retains all student health records for a minimum of seven years beyond majority. KS complies with the Hawai`i Medical Association and American Medical Association rules on medical records confidentiality and retention.
